Horror games aren’t exactly known for delivering 100+ hour playthroughs across gigantic open landscapes, as for the most part, the terrifying adventures are kept confined to a single linear path that normally takes around a dozen hours to get through. However, when looking at all the additional side content, the total time starts to rise pretty far, turning a single playthrough of the game into just a small dent in the actual full competition.
There are collectibles, hidden questlines, challenges, and even entirely new endings that often require a full replay to unlock, amplifying the amount of time it takes to reach that coveted 100% file. If you are willing to stick it out, these games are well worth grinding till the very end, but that assumes you can overcome the fear that remains constant no matter how many times you watch the credits roll.

Look Outside
Completionist Time — 52.5 Hours

Look Outside is the kind of game that really rewards you for going the extra mile and not rushing through every last segment of the story. Set in a run-down apartment building during an apocalyptic event, you spend the entire runtime exploring different floors and encountering a variety of wacky NPCs, from mutated painters to people stuck inside metal tubes, until eventually, you reach one of the 13 possible endings.
That fact alone means that you effectively need to play through the game a dozen times to see everything, and that discounts all the minute decisions that you need to make to tick off all the additional content hiding in the corners of every last apartment. Combined with the grotesque and terrifying creatures that serve as the main obstacles along your journey, and suddenly, a single 20-hour playthrough can easily be tripled.

The Letter – Horror Visual Novel
Completionist Time — 57.5 Hours

The Letter is far more than a traditional visual novel. Its extensive cast, interconnected storylines, and huge number of branching choices mean that seeing every route requires dozens of hours spread across multiple playthroughs, and that’s assuming you can even figure out the correct paths to take that second time around.
Much of that playtime comes from uncovering every possible fate for each character. Different decisions unlock entirely new scenes and even endings, and this in turn makes completion less about reaching the credits and more about experiencing every variation of this unique yet still terrifying supernatural horror story.

The Forest
Completionist Time — 58.5 Hours

- Exploring the whole map takes considerable time
Although it’s possible to complete The Forest relatively quickly, experiencing everything the island offers is an entirely different challenge. You can easily spend dozens of hours constructing elaborate bases and crafting equipment, then venturing out to discover hidden bunkers and caves scattered across the map that almost always house an unexpected twist.
The game’s survival systems naturally extend its lifespan, as you need to invest a huge amount of time just setting yourself up; otherwise, I can guarantee that when night comes, death is just around the corner. And, after overcoming every hurdle and uncovering the mystery surrounding the island itself, there is always more content to absorb, from lore to physical locations that continue raising that overall time.

Resident Evil 4 (2023)
Completionist Time — 65 Hours

Resident Evil 4‘s remake offers exceptional replay value beyond its excellent campaign, which in itself will take many players close to 20 hours to clear. Unlocking every weapon, costume, accessory, and bonus item requires completing the game under strict conditions, including speedruns, minimalist runs, and the punishing Professional difficulty, all classic Resident Evil tropes that have found their way into many of the more modern titles and made them quite the marathon to get through.
Mercenaries mode adds even more content for completionists, while achievement hunting pushes you to master every aspect of the combat system, incorporating parries and skillful snipes to tick off the trickier ones on the list. By the end, you may be exhausted, but I can promise you that Leon’s quirky lines and the satisfaction that comes from clicking heads never seem to get old.

Fear & Hunger 2: Termina
Completionist Time — 66 Hours

Fear & Hunger 2: Termina is deliberately designed around discovery, and because of that, you will end up investing a huge amount of time effectively following the same paths over and over. The branching nature of the progression and the different protagonists to play as ensure that players rarely uncover everything during a single playthrough, assuming they can even make it past the first few barriers.
Also, many of its biggest secrets require experimentation rather than straightforward exploration, so a hidden boss or a rare event can only really be encountered with enough luck in a thorough playthrough. But, as long as you can stomach the brutality, I can honestly say you will be rewarded with one of horror’s most intricate worlds.

OMORI
Completionist Time — 71.5 Hours

- Multiple routes reveal completely different perspectives
OMORI’s emotional story is only part of what makes it so lengthy to fully complete. Full completion means exploring every optional area and defeating every hidden boss, then collecting every item, and experiencing both major story routes, boosting that initial runtime into the stratosphere.
The other dilemma is that its alternate paths fundamentally change how you interpret the narrative, making additional playthroughs feel almost mandatory rather than repetitive and something genuinely worth going for aside from just hitting 100%. A psychological nightmare at first, but a surreal and beautiful experience that everyone should play through at least once.

Pathologic
Completionist Time — 80.5 Hours

- Full narrative only revealed by beating each campaign
It is far from a traditional horror game, but Pathologic is easily the longest and most demanding horror game you can find out there. For starters, just knowing how to get started and progress feels like a mountainous hurdle, and from there, the struggles only seem to get worse, with the plague continuing to spread and your perception of what’s happening becoming more and more warped as time goes on.
The game features three playable protagonists, each experiencing the same town from a completely different perspective, and even after finishing one route, huge portions of the narrative remain unexplained until the other two stories are completed. In my eyes, the true fear comes from failure, and for that reason, I would argue that Pathologic is one of the scariest and mentally taxing horror games around, and a real testament to how powerful world-building can be in the context of fear.
